1.__Formal declarations.__Formal declarations of the claimant or any other individual;

2.__Informal statements.__Informal statements of the claimant or any other individual;

3.__Place of employment.__The place of employment of the claimant;

4.__Previous permanent residence.__The previous permanent residence of the claimant and the date the previous permanent residency was terminated;

5.__Voter registration.__The place where the claimant is registered to vote;

6.__Driver's license.__The place of issuance to the claimant of a driver's license and the address listed on the license;

7.__Certificate of motor vehicle registration.__The place of issuance of a certificate of registration of a motor vehicle owned by the claimant and the address listed on the certificate;

8.__Income tax returns.__The residence claimed on any income tax return filed by the claimant;

9.__Motor vehicle excise tax.__The place of payment of a motor vehicle excise tax by the claimant; and

10.__Military residence.__A declaration by the claimant of permanent residence registered with any branch of the Armed Forces of the United States.

§6224.__Claim is personal

The right to file a claim under this chapter is personal to the claimant and does not survive the claimant's death, but the right may be exercised on behalf of a claimant by the claimant's legal guardian or attorney-in-fact. If a claimant dies after having filed a timely claim, the amount thereof must be disbursed to another member of the household as determined by the assessor.__ If the claimant was the only member of a household, the claim may be paid to the claimant's personal representative,
