Amend the bill in section 1 in paragraph A by striking out all of the last sentence (page 1, lines 8 and 9 in L.D.) and inserting the following: ' A general release form with a separate signed or initialed check box for explicit release of HIV infection status information is sufficient to meet the requirements of this subsection.'

Amend the bill in section 2 in paragraph B in the last 2 lines (page 1, lines 16 and 17 in L.D.) by striking out the following: " or check box on a form" and inserting the following: ' with a separate signed or initialed check box for explicit release of HIV infection status information'

SUMMARY

The bill provides that a check box on a general release form is sufficient for a patient to authorize the release of HIV infection status information. This amendment, which is the minority report of the committee, requires that the check box be separately signed or initialed.
